School-based health clinic coming to Coffee Middle School campus

Coffee Middle School would like to welcome to its campus the new Coffee Regional Hospital (CRH) School Based Health Clinic (SBHC).

During our January in-service, Ben Floyd, Director of CRH Physician Practices, Jennifer Davis, Nurse Practitioner for CRH Physician Practices and Kathy Cole, R.N. Nurse Manager/Telemedicine Director for Coffee County Schools explained the operations for the new CRH SBHC that will open Feb. 1 at Coffee Middle School.

Coffee County Schools/Coffee TeleHealth and CRH Physician Practices have collaborated and partnered to expand access to community health and bring on-site healthcare into our schools. Students, as well as school employees, may receive services. Potential patients must opt in by first filling out the Coffee TeleHealth paperwork.

Mrs. Jennifer Davis, NP will be the on-site provider. She will be available to CMS students, staff and serve the additional 11 schools via Telemedicine from 7:30a – 4:00p Monday–Friday (excluding holidays and weekends).

The packets are available on the Coffee County Schools website at www.coffee.k12.ga.us under TeleHealth/School Nurse Program.

3.    Complete packet

4.    Print completed packet and Sign it

5.    Give completed packet, copy of picture ID and copy of insurance card to the School Nurse
