Mr. Jerry Wayne Leavens, age 75, of Douglas, passed away Sunday afternoon, September 12, 2021 at Coffee Regional Medical Center following a short illness. Mr. Leavens was born June 7, 1946 to the late Otto Leavens and the late Thelma Roberts Leavens. Other than his parents, he was preceded in death by his wife Betty Daniels Leavens.

Mr. Leavens, a native of Douglas, GA was a self employed diesel mechanic for many years. He was also a veteran of the United States Army during the Vietnam War.

Survivors include:

1 Daughter: Tammy Louise Leavens and special friend Junior Gilbreath of
Douglas, GA

Fiance: Sherry Surrency of Adel, GA

3 Grandchildren: Jamie Nicole Harper and husband Rusty of Broxton, GA
Amber Renee Swanson and husband Cameron of Douglas, GA
Angela London Gandy and special friend, Justin Moore of
Lakeland, GA

2 Step-Grand Children: Jim Barkley and Serina Shayene Tomberlin both of
Douglas, GA

9 Great Grand Children

A number of nieces and nephews, cousins and many friends

The graveside memorial service will be held Thursday afternoon, September 16, 2021 at 3:00 PM at Carver Cemetery.

Due to COVID-19, the family ask that all attendees, please follow the rules of social distancing, and wear your mask for safety precautions at the service.

Ricketson Funeral Home is in charge of the arrangements.
