Coffee Regional Medical Center is proud to announce that CRH Family Medicine Group, under the care of physicians Dr. Thaihang Nguyen & Dr. Lydia Tanner, has achieved Level 3 Patient Centered Medical Home Accreditation (PCMH) from the National Committee for Quality Assurance (NCQA). Measuring quality and improving health care is the goal of NCQA. Rarely achieved on the first attempt, Level 3 is the highest level of accreditation.
The PCMH model is associated with happier staff and happier patients. This is the most widely-used PCMH program with more than 12,000 primary care practices (and 60,000 clinicians) across 43 states embracing the program. The program is a powerful tool for transforming primary care into what patients want it to be. It’s a model that puts patients at the forefront of care by building better relationships between patients and the teams who care for them.
The program features a set of six concepts that make up a medical home developed from evidence-based guidelines and best practices. To earn recognition, PCMH practices meet a set of clear, specific criteria that show clinicians how to organize care around patients and work in teams to coordinate, track and improve care. These concept areas include: Patient Centered Access, Team-Based Care, Population Health Management , Care Management and Support, Care coordination and Care Transitions, Performance Measurement and Quality Improvement.
Patient-Centered Medical Homes are driving some of the most important reforms in healthcare delivery today. A growing body of scientific evidence shows that PCMHs are saving money by reducing hospital and emergency department visits, mitigating health disparities, and improving patient outcomes. In addition to benefitting patient care, the PCMH model has tangible benefits to practices and its staff. Practices can align with where health care and payers are headed, and clinicians can see benefits like enhanced reimbursement and the ability to better focus on patient care.

By measuring quality, NCQA gives people the information they need to find high-quality health care. Health care quality means getting the right care in the right amount at the right time.
Right care – This means treatments that science shows actually work. It also means treatment that fits the patients values, lifestyle and circumstances. Right amount – Not getting enough of the care you need is a problem of “too little.” Getting treatment for a condition that goes away on its own is an example of “too much.” Getting multiple scans from doctors who don’t share information is an example of overuse too. Right time – Timing matters because often the best treatment is care that keeps you from getting sick in the first place.
