The Coffee County Health Department will host a Back-to-School clinic on Wednesday, July 22, 2026, from 7:30 AM – 5:30 PM. During this clinic, the health department will provide vaccines, along with hearing, vision, and dental screenings, to fulfill requirements for Georgia DPH Forms 3231 and 3300 for school attendance in Georgia. No appointment is necessary.
The Coffee County Health Department (located at 1111 West Baker Highway in Douglas) is part of the Georgia Department of Public Health’s Southeast Health District.
